New Dealer Application Forms

To become a dealer for PAC products, you have to be a part of the 12 volt industry with a valid business license.

Sales Tax Exempt Certificate - Please fill this form completely and fax it to (800) U-FAX-PAC (800-832-9722) with a copy of your business license and resale certificate.

Credit Application – If you would like Net 30 terms or COD company check. The processing of your application will take a little while because we have to wait to hear back from the vendors.

Credit Card Authorization – If you need the products sooner you can either use a credit card or COD guaranteed funds (cashiers check or money order.) A Credit Card Authorization form is a form that we keep on file for your own protection.

Online Store Notice - If you plan on selling any PAC products online, this form needs to be filled out.

RA Form – Please use this form to request an RA for items being returned. * Heavy bordered areas denote office use only.

There is no minimum purchase with PAC, although, we do have dealer incentives. At $500 or more of PAC products you can get free ground shipping, $600-$999 you can get 5% off your PAC products on order and at $1000 or more you can get 10% off your PAC products on your order.
